CAF Fest Booth Invoice

Below is the information required to secure a booth.  Booth fee is $30.00 for food vendors,  $75.00 for non-food vendors and $100 if you plan to sell your wares.  This is not an online form.  Please fill out this form and email it to Matt Womack at matt@watkinsinsurancegroup.com or you can fax it to him at: 512.452.09999.  For payment, you can pay online at http://www.chooseaustinfirst.com or make your checks payable to: Choose Austin First.  Please send the check to: Matt Womack, 3834 Spicewood Springs Rd, Ste 100, Austin, TX, 78759.   You MUST be a current CAF or AIBA member or a Triangle tenant to participate.
